Antony Jacob, CEO—Apollo Munich, announced that there is 75% of people paying out medical treatment expenses from their pockets, which is perhaps the maximum in the world. As per him, every health insurance company faces the same issue that people in India are unaware of health insurance and the foremost reason is that health insurance is a unprofitable sector and the number of insured people are those who have either faced urgent needs of health insurance themselves or their family member. So, these are the individuals who are purchasing health insurance but it has resulted in high frequency and high cost. Thus, it is becoming unprofitable for health insurance.

As per Antony Jacob, the number of insured people should be a mix of both, healthy and unhealthy people to make it bigger and broader. Thus, there is much improvement to be made in this field.

Apollo Munich has entered into health insurance as a pure health insurance company and he believes that all associated bodies should play an active role in spreading awareness. Regulation should invite more players to focus on this sector, so that they can work morning and evening to encourage people to buy health insurance. It will help to design products, as per the needs of customers, thus contributing towards insurance sector. So, there is a significant role for the regulator to play in terms of formulating rules and regulation, entries and barriers for health insurance. It is to ensure that right players enter the market. Similarly, there is a role for Government to play to spread health awareness and healthcare costs so that people understand that financing through insurance is absolutely a necessity.

Mr. Antony Jacob further said that Apollo Munich has a big role to play in terms of adverting, product innovation and service offerings. Likewise, a big role is to be played by health care providers in terms of standardization of rates and processes. It is the matter of increasing 150 million, pupation to 400 million. There are many government programmes, such as RSBY and they are doing a great job. Private sector has encouraged new entries like Apollo Munich as standalone licensed player.

Comparison of the health care systems in Canada and the United States are often made by government, public health and public policy analysts. The two countries had similar health care systems before Canada reformed its system in the 1960s and 1970s. The United States spends much more money on health care than Canada, on both a per-capita basis and as a percentage of GDP. In 2006, per-capita spending for health care in Canada was US$3,678; in the U.S., US$6,714. The U.S. spent 15.3% of GDP on health care in that year; Canada spent 10.0%. In 2006, 70% of health care spending in Canada was financed by government, versus 46% in the United States. Total government spending per capita in the U.S. on health care was 23% higher than Canadian government spending, and U.S. government expenditure on health care was just under 83% of total Canadian spending (public and private).

Studies have come to different conclusions about the result of this disparity in spending. A 2007 review of all studies comparing health outcomes in Canada and the US in a Canadian peer-reviewed medical journal found that “health outcomes may be superior in patients cared for in Canada versus the United States, but differences are not consistent.” Life expectancy is longer in Canada, and its infant mortality rate is lower than that of the U.S., but there is debate about the underlying causes of these differences. One commonly-cited comparison, the 2000 World Health Organization’s ratings of “overall health service performance”, which used a “composite measure of achievement in the level of health, the distribution of health, the level of responsiveness and fairness of financial contribution”, ranked Canada 30th and the U.S. 37th among 191 member nations. This study rated the US “responsiveness”, or quality of service for individuals receiving treatment, as 1st, compared with 7th for Canada. However, the average life expectancy for Canadians was 80.34 years compared with 78.6 years for residents of the U.S.

The WHO’s study methods were criticized by some analyses. Some argue that Canada has had higher mortality rates for some conditions, such as heart disease, owing to the use of particular medications. Although there is a measure of consensus that life-expectancy and infant mortality mark the most reliable ways to compare nation-wide health care, a recent report by the Congressional Research Service carefully summarizes some recent data and notes the “difficult research issues” facing international comparisons.

The health care system in Canada is funded by a mix of public (70%) and private (30%) funding, with most services delivered by private (both for-profit and not-for-profit) providers.

Through all entities in its public-private system, the U.S. spends more per capita than any other nation in the world, but is the only wealthy industrialized country in the world that lacks some form of universal health care. In March of 2010, the US Congress passed regulatory reform of the American health insurance system. However since this legislation is not fundamental health care reform, it is unclear what its effect will be and as the new legislation is implemented in stages, with the last provision in effect in 2018, it will be some years before any empirical evaluation of the full effects on the comparison could be determined.

Health care costs in both countries are rising faster than inflation. As both countries consider changes to their systems, there is debate over whether resources should be added to the public or private sector. Although Canadians and Americans have each looked to the other for ways to improve their respective health care systems, there exists a substantial amount of conflicting information regarding the relative merits of the two systems. In Canada, the United States is used as a model and as a warning against increasing private sector involvement in financing health care. In the U.S., meanwhile, Canada’s mostly monopsonistic health system is seen by different sides of the ideological spectrum as either a model to be followed or avoided.

India has only about 15% of the population covered under health insurance. He believes that there is still a lot that needs to be done to increase the pool of people who are involved in Health Insurance. About 75% of the people in the country spend money out-of- pocket on healthcare, which is probably the highest in the world.

Antony Jacob said that there are very few people who buy health insurance. He added that the trend is that health insurance is mostly bought by people after an urgent need for treatment or healthcare is felt; many-a-times this need is felt by family and friends of people who require treatment. He said that this was the reason for the lack of profitability of the health insurance sector in the country. There should be a balance in the industry. This makes the scope of health insurance much wider and thus far more profitable.

He further added that there are many factors that need to be addressed in order for health insurance to be popularized. Apollo Munich has made its way into the market as a private sector specialist health insurance provider.

According to him, the recent regulations should help many other companies come into the market which would solely focus on health insurance. These companies would not be dealing with any other work and therefore their entire concentration would be streamlined on medical insurance and the changing trends of healthcare. This would enable them to bring out better products.

He also said that there is an important role for the regulators to play in terms of promoting pure health insurance companies. The government too has potential of playing a strong role in promoting awareness about current and future healthcare costs and how insurance is important. There is also a major part to be played by insurance providers in line with their branding and products. The healthcare industry too can contribute by ensuring that healthcare costs are regulated and procedures of healthcare are standardized.

According to Mr. Jacob, it is only when all these segments come together that the goal of health insurance companies would be realized. With the private sector being encouraged, new players are making their way into the market and will promote the same. The currently 150 million covered individuals will soon grow to about 400 million people.
